Gross overcrowding, a lack of available staff and an emergency department that didn't give patients privacy or dignity.
Those were among the findings of three HIQA inspections late last year in Kerry University Hospital, Tallaght Hospital and Cork Maternity Hospital.
The Irish Nurses and Midwives Organisation says of 10 reports written by inspectors since last March, not a single hospital has passed safe staffing requirements fully.
It's called on both the HSE and Department of Health not to allow the reports to 'sit on a shelf' and has re-iterated its demand for action on overcrowding.
